DECISION ON JOINT STIPULATION1 On September 12, 2023, Fadia Louala filed a petition for compensation under the National Vaccine Injury Compensation Program, 42 U.S.C. §300aa-10, et seq.2 (the “Vaccine Act”). A lump sum of $75,000.00, to be paid through an ACH deposit to Petitioner’s counsel’s IOLTA account for prompt disbursement to Petitioner; and b. A lump sum of $6,423.53, which amount represents reimbursement of a Medicaid lien for services rendered on behalf of Petitioner by the State of Arizona, in the form of a check payable jointly to Petitioner and Optum PO Box, 182643 Columbus.
